Thank you for booking! Before your upcoming visit, we want to welcome you  and ask that you please fill out the information in the form below. 

 

Our insurance company and the State of Florida mandate that we have this information on file. Florida Statutes 509.101 (2): It is the duty of each operator of a transient establishment to maintain at all times a register, signed by or for guests who occupy rental units within the establishment. Property will not be occupied by more than eight (8) people, at any one time; all tenants are listed on the form below.

 

Our property is protected by an Electronic Lock. To access our property, you will need to use the Access Code that we will provide to you before your arrival. Check in time is at 4 p.m. and ends at 10 a.m. on the check-out date listed in the reservation. 

• We will need this information from you before check in Date of the reservation.

• Full names and ages of all Guests.

• Your mailing and credit card address if different. 

• Driver’s License Number and State of issue.

• Credit Card number, CVV # (3 digit code on back of card) expiration date and the billing address of the card. This will only be used for unusual damage or incidentals such as power allowance overages.

 

Guest further understands and acknowledges that homeowner can charge credit card on file for any person not listed that is considered staying at the property as an overnight tenant and for incidentals such as electricity allowance.

Note: Electrical consumption is NOT included in the rental rate. Consumption at the rate of 13 cents per kilowatt will be billed to Guest within 10 days of departure.

All Guests of our properties receive a concierge visit generally on your day of arrival. Our friendly property management staff will meet you at the home to show you all of the amenities and answer any questions for you. They will be glad to make recommendations on activities, restaurants, or other area interests for our guests to ensure you have a fantastic vacation.
